Community Rowing Swim Test Form

Every rower at CRI must have a Swim Test on file with the office in order to row. If you do not have a Swim Test you MUST wear a life jacket while on the water.

CRI Participant Health Release Form

Every participant must have a Participant Health Release Form on file before the first day of the program. This form provides CRI with emergency contact information and is filed in our main office for the season.

CRI Code of Conduct Contract
For any youth that will be traveling with the team to competitions.  It is required that every athlete renews this form with the club each year.

CRI General Programs Waiver

If you are visiting the club for a day or taking part in a special program for our insurance it is required that everyone fill out our waiver.

CRI Youth Medical Travel Form
For any youth traveling with the team on training trips or to competitions.  It is very important that we have up to date medical information on every athlete.

Alternate Travel Release Form
For any youth who plans to return from a regatta separately from the team. Must be completed three days before competition and turned into coaches.

CRI Incident Report
After any incident whether on the water, in the boathouse, on the dock, or in the parking lot it is necessary to fill out an incident form.  Forms can be found at the front desk to fill out at the time of the accident.  Please return forms to the front desk.
